FULL MEDICALS AND CLEARANCE HAS STARTED

STEP 1... Walk into the school premises and ask for the Memorial Medical Center. Show them your clearance form with your acceptance fee receipt. After signing, you will be giving the documents showing the hospital you will go for a checkup. You will bring it back to school when done.

STEP 2... Students should locate their various departments and ask for school officer. The school officer will guide you on how to arrange your files and what is needed. After completion of arrangement of files, he will sign.

STUDENTS ARE TO MAKE 3 COPIES EACH OF THESE DOCUMENTS

• >ORIGINAL O.LEVEL RESULT WITH (3 PHOTOCOPIES)
• >ORIGINAL JAMB RESULTS WITH (3 PHOTOCOPIES)
• >ACCEPTANCE FEE RECEIPT. ( 3 PHOTOCOPIES)
• >CLEARANCE FORM
• >TEN PASSPORT PHOTOGRAPHS

All students that printed their jamb original result before doing the change of institution are to print a fresh jamb original result that will show Mapoly or Ogun state polytechnic. Please cross check your jamb original slip.
